Clinical Capabilities
A specialized environment ready to accept your complex neurological discharges.
Supported Acuity Levels
Unlike standard assisted living or lower-tier nursing homes, our RN and LPN staffing ratios allow us to safely manage patients with severe deficits and intense intervention needs.
- Advanced Wound Care
Management of Stage III/IV pressure ulcers, surgical wounds, and specialized dressings.
- Enteral & Parenteral Nutrition
Continuous and bolus G-tube/J-tube feeds, managed by registered dietitians.
- Complex Neuromuscular Deficits
Total lift, Hoyer, and ceiling-track transfers for paraplegia and quadriplegia.
Exclusions
To maintain a safe environment and ensure we can meet our residents' needs, there are specific clinical profiles we cannot accept for admission.
- Ventilator Dependent (trach collars accepted case-by-case)
- Active, severe psychosis or violent behavioral disturbances
- Wandering dementia risks (non-secure perimeter)
- IV Push Medications (some IV antibiotics accepted)
Facility Infrastructure & Equipment
High-acuity care requires the right physical environment. Our facility is heavily capitalized to maintain a hospital-grade infrastructure in a home-like setting.
Centralized Oxygen
Wall-integrated O2 and suction in resident rooms, eliminating portable tanks.
Bariatric Support
Heavy-duty beds, ceiling lifts, and widened doorways for specialized needs.
Low-Air-Loss Surfaces
Advanced pressure-relieving mattresses standard for immobile residents.
In-House Diagnostics
Mobile X-ray, lab draws, and customized wheelchair seating clinics on campus.
The Interdisciplinary Cohort
Care strategy is not dictated in a silo. We utilize a true interdisciplinary model, rounding weekly to adjust care plans based on slight changes in clinical presentation.
Attending Physicians
Led by our Medical Director, with regular rounding and 24/7 on-call availability for urgent interventions.
Specialist Consults
Close coordination with local neurologists, pulmonologists, and physiatrists for disease-modifying therapies.
Therapy & Nursing
Seamless communication between wound care nurses, physical therapists, and speech-language pathologists.
Fast-Track Pharmacy Integration
Neurological patients often have highly sensitive, multi-drug regimens where missed doses lead to severe consequences. Our pharmacy partners are integrated directly into our EMR.
On-site automated dispensing cabinets for immediate access to stat orders and newly prescribed antibiotics.
Rigorous admission med-reconciliation to prevent adverse interactions during transition of care.
