A Trusted Partner in Complex Discharges
We understand the challenges case managers and discharging physicians face when transitioning high-acuity neurological patients. The Beechwood Home offers the specialized clinical infrastructure to safely manage these cases and prevent hospital readmissions.
The Ideal Admission Profile
We are not a standard geriatric nursing facility. Our environment, staffing, and architecture are specifically calibrated for adults battling chronic, progressive neurological diseases or recovering from devastating spinal and brain injuries.
- Primary Diagnoses
Multiple Sclerosis (MS), Amyotrophic Lateral Sclerosis (ALS), Huntington's Disease, Parkinson's, Cerebral Palsy, severe Stroke, and Spinal Cord Injuries (SCI).
- Acuity Level
Patients requiring heavy assistance with ADLs, specialized seating/positioning, Hoyer lifts, complex medication management, and dysphagia protocols.
- Age Demographic
Because neurological diseases often manifest early, our population tends to be younger than traditional care facilities, creating a more dynamic peer environment.

Our Clinical Scope of Practice
Unlike traditional long-term care facilities, we staff heavily to manage severe medical complexities, allowing you to confidently transition patients out of acute care.
Advanced Nutrition
Comprehensive management of enteral feeding (G-tubes, PEG tubes), complex dietary formulations, and strict dysphagia protocols.
G-Tube / PEG CapableWound & Skin Integrity
Aggressive protocols for Stage III and IV pressure ulcers, utilizing advanced mattress technology, wound vacs, and specialized nursing care.
Wound Vac CapableInsurance & Payer Alignment
We maintain active standing with major regional and national payer sources to expedite the financial approval process prior to admission.
- Ohio Medicaid
- Medicare Part A & B
- Commercial Insurance
- Medicaid Managed Care
- Veterans Affairs (VA) Contracts
Exclusion Criteria
To protect the safety of our resident population and ensure we are utilizing our highly specialized neurological beds appropriately, our Director of Nursing strictly screens against the following clinical profiles:
Wandering Dementia
We do not operate a locked memory care unit. Patients with severe Alzheimer's or frontotemporal dementia who exhibit wandering behaviors are not clinical fits.
Active Addiction/Violence
Patients with primary diagnoses of active substance abuse or a documented history of severe physical violence towards caregivers or peers cannot be admitted.
Ventilator Dependence
Our facility is not currently certified or equipped for long-term mechanical ventilation dependency.
Active Infectious Disease
Certain severe airborne active infectious diseases that require negative-pressure isolation environments cannot be accommodated safely.
Preventing Readmissions
Direct Physician Oversight
Our highly involved Medical Director conducts routine rounds and is heavily integrated with the UC Health and TriHealth neurological teams.
In-House Therapeutics
Immediate access to PT, OT, and SLP prevents the physical deconditioning that often triggers 30-day hospital bounce-backs.
High RN Staffing Ratios
As a non-profit, we mandate higher nurse-to-resident ratios, allowing for immediate identification of subtle clinical changes like UTIs or pneumonia.
Referral Process
To initiate a clinical review for discharge, please fax the patient's face sheet, complete H&P, recent nursing/therapy notes, and medication list.
Rapid Clinical Review typically within 24 hours.
Measured Clinical Outcomes
We do not rely solely on our reputation; we measure our efficacy. Because our nursing staff is specialized in recognizing the early onset of neurological exacerbations (such as aspiration pneumonias or MS pseudo-exacerbations), we are able to intervene in-house before the patient requires an ambulance.
< 8%
30-Day Readmission
Significantly lower than the national average for high-acuity neurological skilled nursing discharges.
5-Star
CMS Quality Rating
Consistently achieving the highest possible rating for quality measures from the Centers for Medicare & Medicaid Services.

Multidisciplinary Integration
Siloed care models fail medically complex patients. At The Beechwood Home, care is executed through a tight, Interdisciplinary Team (IDT) model. We ensure that a patient's entire clinical picture is assessed and managed synergistically.
- Daily Stand-Ups
Nursing, Dietary, Social Work, and Therapy meet every morning to discuss changes in patient condition.
- Palliative & Hospice Integration
We partner seamlessly with regional hospice agencies to provide uninterrupted end-of-life care within our facility.
- Psychological Support
Access to clinical psychologists focusing on the profound mental health challenges associated with degenerative disease.
Regional System Integration
The Beechwood Home is a trusted, long-standing post-acute partner for the Greater Cincinnati health systems. We actively collaborate on education, research, and seamless patient transitions with the leading neurology, pulmonology, and physical medicine departments across the tri-state area.
